Surgical treatment of severe acromioclavicular separation continues to evolve. Surgical reconstruction is recommended for type IV and V injuries and symptomatic type III injuries that fail nonoperative treatment. Many surgical techniques have been developed, including primary ligament repair, modified Weaver-Dunn reconstruction, and free soft tissue transfer passed either through or around the coracoid process.
