These forms will help to make your registration process faster and eliminate some of the paperwork you would normally fill out in the office before seeing the doctor. You can print the form(s), complete each one and bring it with you to your appointment. If you forget to bring the forms with you, you will have to fill them out in the office before the doctor can see you.

Payment is due in full at the time of the visit for all services, unless the patient is enrolled in a managed care plan in which KSF Orthopaedic Center participates. All co-payments and co-insurance information are collected from managed care patients at the time of their visit. Patients may pay for services provided with cash, check, Visa, Master Card, Discover or American Express.
